Case 73

Blount Disease with corrective osteotomies

Findings

There are bilateral abnormalities of the proximal tibial metaphyses and epiphyses, more so on the left. There is flattening and irregularity of the medial aspects of the epiphyses, resulting in bilateral tibia vara. The bands of sclerosis in the upper tibial shafts are the sites of corrective osteotomies.

Diagnosis

Blount disease of the tibial plateaux with bilateral corrective osteotomies.

Discussion

Blount disease is thought to be one of the osteochondritidies, which affects children in the first decade. It results in deformity of the medial tibial metaphysis, with medial and posterior beaking, often complicated by deformation of the medial epiphysis. The result is tibia vara (lateral bowing of the legs), which may require corrective surgery, as here.
