Progressive massive fibrosis
A retired man with chronic breathlessness
The following chest film was obtained.
Findings
The appearances are those of progressive massive fibrosis, a late complication of coal worker's pneumoconiosis. There is progressive fibrosis affecting predominantly the posterior segments of the upper lobes and apical segments of the lower lobes, resulting in perihilar fibrotic masses which slowly move migrate towards the hila.

Discussion
The masses usually have sharp outer margins but ill defined inner borders. As the lesions progress, the nodularity secondary to simple pneumoconiosis may reduce, as nodules are "consumed" within the masses, and compensatory emphysema develops more peripherally. Note the lack of lung markings in the right upper zone in this case.
There is freqently evidence of coexistant pulmonary tuberculosis, which may explain the calcified nodules.
